Playing Out

I consent to the above-named child attending open access play activities on the grounds around St. Andrew’s Methodist Church on the 23rd / 30th July and 6th / 13th August between 1pm-3pm.*

I understand that the project will be staffed by DBS checked volunteers and will include access to sports equipment and craft activities. I understand my child may enter the church building (under supervision) to access the toilets or to shelter from heavy rainfall.*

I give my consent for volunteers to administer basic first aid as required and to act on my behalf with emergency healthcare practitioners in my absence if I cannot be contacted.*

I understand that children are free to come and go from the provision as they wish however in the event of a child under the age of 8 (or assessed as being vulnerable) wishing to leave unsupervised, efforts will be made to keep them onsite until a parent / guardian can be contacted.*

The data contained on this form will be destroyed at the end of the summer and will not be used for any other purpose however a record of children attending will be securely retained in line with our Children's Safeguarding policy.
